Become a Patient in 3 Easy Steps

Speak with a Patient Advocate

Submit Medical History and Health Insurance Form
Your Patient Advocate will send you a link to our online Medical History Form. Once submitted, your Patient Advocate will call you to confirm we have received your form. It typically takes about 24-48 hours to determine your eligibility for stem cell therapy. Once a decision is made your Patient Advocate will contact you and help answer any further questions.

Schedule Your Appointment
If your Medical History Form is approved and you’re interested in coming for stem cell therapy, your Patient Advocate will schedule your 2-day appointment. Our Care Coordinator will then call and arrange your travel.
Travel
Your Care Coordinator will handle your trip details which includes ground transportation to and from the airport along with hotel accommodations for you and your loved ones.
We will then mail you out a personalized packet of information and our nurse will call you to discuss your medical history also helping to discuss any further questions you may have prior to your visit.
